At some point we are all likely to need support with our health and wellbeing so we can live well and independently at home or close to home.  An increasing number of people in Moray - just like the rest of Scotland - live with more than one health problem and need the support of family, friends, their community, health and social care services. Often services don't work together as well as they could and there are too many gaps.

Integration is the Scottish Government's ambitious programme of reform to improve services and outcomes for everyone who uses health and social care. Health & Social Care Moray is a partnership comprising of The Moray Council and NHS Grampian and which is governed by an Integrated Joint Board.

Joining up adult services to provide seamless, person-centred care at the right time and in the right place, will help individuals and their families live fuller lives and improve their experiences of care and support.  This will be most important for those with complex needs involving support from health and social care at the same time.
